Ischias entsteht meist durch Druck auf den Ischiasnerv, oft durch einen Bandscheibenvorfall in der Lendenwirbelsäule. Typisch sind ausstrahlende Schmerzen vom unteren Rücken bis in den Fuß, begleitet von Taubheitsgefühlen oder Schwäche. Beim Piriformis-Syndrom hingegen verkürzt sich der Piriformis-Muskel tief im Gesäß und klemmt den Ischiasnerv ein. Hier liegen die Schmerzen eher im Gesäßbereich und verstärken sich beim Sitzen oder Treppensteigen.

Wie Sie die beiden selbst besser einschätzen können

Führen Sie den Piriformis-Test durch: Legen Sie sich auf den Rücken, winkeln Sie das betroffene Bein an und legen Sie den Knöchel aufs gegenüberliegende Knie. Drücken Sie nun das Knie sanft nach außen. Zieht es tief ins Gesäß, spricht das eher für ein Piriformis-Problem. Bei Ischias spüren Sie den Schmerz eher im unteren Rücken oder entlang des ganzen Beins.

Hormonelle Veränderungen in den Wechseljahren verstärken beide Beschwerden, weil Östrogenmangel Muskeln und Bindegewebe schwächt.
